This interactive SIG Session will provide practical strategies to implement or refine social needs screening and referral programs across various pediatric practice sites (including outpatient, inpatient, ED, and ICU settings). Attendees will learn about existing policies and programs that address social determinants of health, as well as frameworks for effectively integrating screening tools into clinical workflows.
The SIG Session will begin with 30 minutes of didactic presentations, offering information on social needs screening across pediatric practice sites, examples of successful programs, and strategies for implementation. This will be followed by 30 minutes of small group discussions, where participants will explore challenges, current practices, and opportunities for improvement in their respective practice settings. This will also be an important networking opportunity. The final 25 minutes will focus on sharing with a broader group and engaging in an “Ask the Experts” session.
By the end of the session, participants will have an understanding of how to address social needs across unique pediatric settings and walk away with actionable plans for improving screening and referral processes. Resources will be provided to support ongoing implementation efforts.
