Workshop Description: The Centers for Medicare and Medicaid Services (CMS) recently mandated that admitted patients and their families must be screened for social determinants of health (SDOH) as a quality measure. As a result, families of pediatric patients who experience food insecurity and other resource limitations are being increasingly identified. Yet there are ethical concerns when at risk families are identified, but no response mechanisms are in place. Using case-based and team-based learning approaches, pediatric providers will gain familiarity with how to care for patients and their families when such resource limitations are identified, and better implement institutional response mechanisms. During this interactive workshop, learners will explore strategies to develop and implement sustainable interventions with seasoned practitioners from multiple institutions across the country and a diversity of practice settings. As a result, they will develop critical advocacy skills such as performing a needs assessment and landscape analysis, mapping stakeholders and aligning with leadership interests, building community partnerships for resource provision, and utilization of monitoring and evaluation data to assess the impact of interventions. Participants will leave the session with an actionable intervention roadmap to respond to resource instability identified through SDOH screening tools in their practice setting, and key resources to bring back to their home institutions.
